Acknowledgement of Return to Play Protocol

Montclair FC has published a comprehensive Return-to-Play Protocol, which is available at www.montclairfc.com/returntoplay or by request from any Club Representative.

Every member must acknowledge they have read and understand the Return-to-Play Protocol and commit to adhere to the protocols established in the plan prior to attending any in-person team activities. 

Montclair FC Member COVID-19 Return-to-Play Acknowledgement and Consent

  • I declare that I have read, fully understand and agree to comply with Montclair FC’s Return-to-Play plan.
  • I am aware and understand details of future stages of the return-to-play will be released prior to advancing that stage. I will review and adhere to these protocols as they are released. 
  • I agree to honestly follow the at home pre-training responsibilities prior to every team function and keep my child home from any team function when experiencing any symptoms of COVID-19.
  • I agree to following the notification protocols and consult a physician when experiencing any symptoms of COVID-19. 
  • I agree to have my child answer honestly to any pre-training screening before every team function.

Medical Release Waiver

As the parent/legal guardian of the aforementioned registered player, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well being of my dependent. If said care requires the above mentioned player to be admitted to any hospital or medical facility for diagnosis and treatment, I hereby in my absence request and authorize physicians, dentists and staff, duly licensed as Doctors of Medicine or Doctors of Dentistry or other such licensed technicians or nurses affiliated with such facilities to perform any diagnostic procedures, treatment procedures, operative procedures and x-ray treatment of the above minor. I have not been given a guarantee as to the result of examinations or treatment. I authorize the hospital or medical facility to dispose of any specimen or tissue taken from the above mentioned player.

Inclement Weather Policy

Montclair FC plays in rain when temperatures are above 50 degrees. While we can’t always predict the weather, we work hard to try to make the best decision for the children. Montclair FC will make every effort to determine the status of class no later than 1 hour before the scheduled start time of class, based on available hourly weather forecasts and field closures beyond our control. We will cancel classes due to adverse weather conditions with short notice or while class is in progress if the safety of the children is in question. If we do cancel a class, we will notify you via email and/or text message.

We will use the email address and phone number we have on file for cancellation updates. Please check your security and spam settings to allow emails from Montclair FC. If you use Gmail, please make sure to check the promotions folder.

Make-Up Classes and Cancellations

  • Montclair FC will hold the week following the last scheduled class as an inclement weather make-up date.
  • Inclement weather cancellations will be made up as follows:
    • 1-2 classes cancelled due to inclement weather = 1 make-up class added to end of season
    • 3-4 classes cancelled due to inclement weather = 2 make-up classes added to end of season
  • In the event of inclement weather on a scheduled make-up class date, there will be no additional make-up dates scheduled.
  • Individual make-up sessions are not provided for failure to attend a scheduled class.
  • Montclair FC reserves the right to cancel any program due to insufficient enrollment. Upon cancellation, a refund for the cost of the season will be provided.
  • If a session or season needs to be canceled for any reason, Montclair FC may provide alternative programming as a replacement.

Photo Release

As the child(ren)’s parent/guardian, I hereby grant a license to the Montclair Soccer LLC and their agents, including any advertising agencies, to use and to license others to use the child(ren)’s name, recorded voice, image, picture or likeness in any live or recorded audio, video or photographic display or other transmission for purposes of promotion and publicity in connection with all Montclair FC programming and any future Montclair FC events or programs and hereby waive any rights of compensation or ownership thereto. 

Field Closures/Cancellations

Montclair FC will not be held responsible for any cancelled games or practices due to field closures. The Prince William County Park Authority makes a determination on a daily basis if fields will be open. The Park Authority’s decision to close fields will dictate if practices and games are cancelled. Due to this, Montclair FC does not guarantee a minimum number of practices and/or games in which players will participate; but efforts will be made to complete all practices scheduled. Field closures/cancellations will be posted on the Montclair FC website.

Refund Policy

At Montclair FC Soccer Club, we understand that circumstances may change, and we strive to provide a fair and transparent refund policy for our members. Please review the following guidelines regarding refunds:

  1. Registration Fees:
    • Refund requests must be submitted in writing via email to info@montclairfc.com.
    • Full refunds, minus a $50 administrative fee, will be granted if the request is made prior to the start of the season.
    • Once the season begins, refunds will be issued on a prorated basis up to the third week of the season, minus the $50 administrative fee.
    • No refunds will be issued after the third week of the season.
  2. Uniform Fees:
    • Uniform fees are non-refundable once the uniform has been ordered. If a refund request is made before the uniform is ordered, a full refund will be granted.
  3. Cancellations by the Club:
    • If Montclair FC cancels a program or season, a full refund will be provided, including all fees paid.
  4. Special Circumstances:
    • In the case of injury or other unforeseen circumstances, refund requests will be considered on a case-by-case basis. Please contact info@montclairfc.com for more information.
  5. Processing Time:
    • Refunds will be processed within 14 business days of the approved request. Refunds will be issued via the original payment method.
  6. Refund Request Procedure:
    • To request a refund, please email info@montclairfc.com with the following information:
      • Player’s Name
      • Parent/Guardian’s Name
      • Reason for Refund Request
      • Proof of Payment
